PGY1 Pharmacy Residency Program The VA Maryland Health Care System (VAMHCS) is seeking TWO highly motivated Postgraduate Year 1 (PGY1) Residency Program candidates. This 12-month ASHP-accredited program is designed to build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. This residency places emphasis on clinical and distributive pharmacy services, using a comprehensive, hands-on, clinically focused approach. Through flexibility and opportunity, this program fosters a unique residency experience, serving a challenging and rewarding veteran population. The VAMHCS PGY1 program offers learning experiences in diverse practice areas including Acute Care, Ambulatory Care, and Long-term Care. Core rotations for this program include: Internal Medicine, Critical Care (MICU or SICU), Cardiology, Psychiatry, Practice Management, Hospital Pharmacy (includes staffing one weekend per month), Long-term care/Geriatrics and Ambulatory Care. A variety of elective rotations are available within the VA Maryland Health Care System including Infectious Disease/Antimicrobial Stewardship, Surgery, Hospice/Palliative Care amongst others. The VAMHCS is a multi-campus health system. Rotations will be held a multiple sites in the Baltimore-metro area such as the Loch Raven and Perry Point campuses. Teaching The Postgraduate Year 1 Residency Program in collaboration with the University Of Maryland School Of Pharmacy offers numerous teaching experiences. Teaching opportunities include lecturing, leading small group discussions, conducting practice lab sessions and precepting Doctor of Pharmacy students. PGY2 Ambulatory Care Residency Program The VA Maryland Health Care System (VAMHCS) at Baltimore is seeking ONE highly motivated Postgraduate Year Two (PGY2) Ambulatory Care Pharmacy Residency Program candidates. This 12-month ASHP-accredited PGY2 Ambulatory Care Residency is designed to develop proficient ambulatory care practitioners with excellent practice skills and leadership abilities to establish new ambulatory care pharmacy programs. Most residents completing this program have been employed as ambulatory care clinical pharmacy specialists in the Veterans Health Care System or comparable health maintenance organizations, while others have chosen to be educators working in academia. AMBULATORY CARE PHARMACY TRAINING The resident will work closely with clinical pharmacy specialists with an advanced scope of practice as well as other practitioners in ambulatory care clinics. The resident is required to participate in the following clinics: Primary Care PACT, Lipid/Risk Reduction, Diabetes, Anticoagulation, Hepatitis C, Women’s Health Clinic, Home-Based Primary Care and COPD group. In addition, residents may rotate through other elective specialty clinics such as: CHF, Cardiology Transitions of Care, Pain Management, Memory Disorders, Interdisciplinary Renal Transplant as well as other potential off-site clinic experiences through affiliated institutions. A major emphasis of the residency is providing direct patient care with the supervision from respective preceptors. Resident responsibilities on rotation include but are not limited to the following: obtaining a pertinent patient history, performing an accurate focused physical exam, assessing pharmacotherapeutic issues and implementing an appropriate pharmacotherapy plan, ordering labs, electronic documentation of progress notes, and providing accurate drug information to patients and other health care providers. Residents are scheduled to 5 to 7 half-day clinics per week with the remaining time available to fulfill other administrative residency responsibilities including research. Clinic assignments will be reviewed in conjunction with the resident and scheduled quarterly to provide a variety of required and elective clinic experiences. A minimum of one required clinic will be scheduled for the duration of the residency year. The resident will develop advanced leadership skills through active participation in clinical service development opportunities as part of a required longitudinal clinical pharmacy administration rotation. VA Maryland Health Care System PGY2 Geriatric Pharmacy Residency Program The VA Maryland Health Care System (VAMHCS) at Baltimore is seeking ONE highly motivated Postgraduate Year Two (PGY2) Geriatric Pharmacy Residency Program candidates. This program is currently under ASHP candidate status. This 12-month PGY2 Geriatric Pharmacy Residency is designed to develop the resident to be proficient in optimization of geriatric pharmacotherapy, performance of medication reconciliation and medication management in older adults, and collaboration with the different roles of the interdisciplinary team in the assessment and management of the older patient. GERIATRIC PHARMACY TRAINING The resident will work closely with clinical pharmacy specialists with an advanced scope of practice as well as other practitioners in geriatric practice areas. The resident is required to participate in the following learning experiences: Long Term Care, Home-Based Primary Care, Memory Disorders clinic, Anticoagulation, Pain clinic, Hospice/Palliative Care, Lipid Risk Reduction Management clinic, Antimicrobial Stewardship, and Internal Medicine. In addition, residents may rotate through other elective specialty practice areas such as: Renal clinic, HF clinic, as well as other potential off-site clinic experiences through affiliated institutions. A major emphasis of the residency is providing direct patient care with the supervision from respective preceptors. Resident responsibilities on rotation include but are not limited to the following: obtaining a pertinent patient history, assessing pharmacotherapeutic issues and implementing an appropriate pharmacotherapy plan as related to geriatric patients, electronic documentation of progress notes, and providing accurate drug information to patients and other health care providers. The resident will be expected to become a integral part of the interdisciplinary geriatric team. OPPORTUNITY Journal club, case presentation, and formal pharmacotherapy presentations are conducted regularly in conjunction with other participating Baltimore residency programs. The resident will participate in select teaching activities at the University of Maryland School of Pharmacy. Teaching activities may include didactic lectures, leading small group discussions, assessing students on clinical skills, and serving as a co-preceptor during experiential rotations. A teaching certificate program is available through the University of Maryland School of Pharmacy in which the resident may pursue. The resident is required to conduct a research or quality improvement project for presentation at the Eastern States Residency Conference and compose a manuscript of publishable quality.
