About Adventist Health White Memorial
Our Hospital: Adventist Health White Memorial (AHWM) is a 353-bed, nonprofit, faith-based teaching community hospital, providing a full range of inpatient, outpatient, emergency and diagnostic services to communities in and near downtown Los Angeles. AHWM l is one of the region’s leading nonprofit hospitals that houses 4 medical, 1 pharmacy and 1 nursing residency programs. Its services include cardiac and vascular care, intensive and general medical care, oncology, orthopedic care, rehabilitation, specialized and general surgery, and women’s and children’s services. Our Pharmacy Department: The Pharmacy Department utilizes the advanced practice of resident-facilitated, pharmacist-driven and patient-centered medication management model, focused in acute care service that is extended to post-discharge car...e for chronic medical conditions. Our ASHP-accredited PGY-1 Pharmacy Residency Program adopts ASHP standards to provide a rich learning and practice-based experiences with comprehensive training in all aspects of pharmaceutical care - knowledge, skills and philosophy - in a collaborative environment. Our Pharmacy Residency: • Our pharmacists and residents work under P&T-approved clinical protocols including but not limited to antimicrobial therapy, anticoagulation, glycemic control, pain management, parenteral nutrition therapy, and transition of care (TOC) service • The residents engage in longitudinal activities such as hospital pharmacy practice, Medication Use Evaluations (MUE), Pharmacy and Therapeutics (P&T), clinical conferences, readmission prevention, preceptorship to APPE students, teaching certification, research & publication, and quality improvement and assurance • Collaborate side-by-side with medical residents in interdisciplinary patient care and teaching rounds including morning reports, daily rounds, and grand rounds. • Engage in various hospital task forces and committees including P&T, Sepsis, Glycemic Management, Medication Error Reduction Program (MERP), Readmission Prevention • Complete a research project to be presented at the Western States Conference • Develop leadership skills through overseeing interns and APPE students • Coordinate events such as accreditation surveys, residency showcases, open house, ASHP matching, and community outreach Our Residency Learning Experiences: • Core rotations: Internal Medicine, Transitions of Care, Infectious Diseases/Antimicrobial Stewardship, Pharmacy Administration, Critical Care, Pediatric • Longitudinal rotations: Staffing, Preceptorship, Teaching certificate, Research and publication Our Clinical Pharmacists, Residents, and Faculty: • 70% clinical pharmacists board-certified in cardiology, critical care, pediatrics, and pharmacotherapy • Provide advanced protocol-driven clinical services in anticoagulation, antiepileptic therapy, antimicrobial stewardship, insulin therapy, erythropoietin therapy, parenteral nutrition, and pharmacokinetic dosing. • Participate in rounds with various services such as geriatrics, infectious diseases (ID), Intensive Care Unit (ICU), Neonatal Intensive Care Unit (NICU). • Attend emergency codes: Stroke, code blue, code peds, code sepsis and code hemorrhage. • Provide Comprehensive Medication Management including Transitions of Care service. The TOC team focuses on discharge drug regimen interventions and post- discharge follow-ups to reduce readmissions.
