Northwest Medical Homes (previously known as Springfield Family Physicians) is an independent Family Medicine primary care practice located in Eugene and Springfield, Oregon. We have been recognized by the Oregon Health Authority as a five star Patient Centered Primary Care Home. The pharmacy department includes an anticoagulation clinic, a transition of care team, and a quality team including Medicare Annual Wellness Visits.

Training Experience:
Work collaboratively with physicians, physician’s associates, nurse practitioners, psychologists, other health care professionals, and office staff to prevent and alleviate medication related problems and optimize patient outcomes.
Provide patient-centered, evidence-based care for complex patients with chronic conditions.
Effectively educate patients and staff, precept pharmacy students, and lead clinical topic discussions.
Perform Medicare Annual Wellness Visits and work alongside our Quality Team to focus on achieving insurance metrics.
Experience the administrative operations of an independent, physician owned, primary care clinic.
Learn to speak the language of billing, insurance contracts, and reimbursement models.
Gain the knowledge and skills required to market yourself to any primary care clinic in the country so that you can create your own ambulatory care practice upon residency completion.

Northwest Medical Homes is the largest independently owned Family Medicine practice in Lane County, Oregon. It consists of three clinics: Springfield Family Physicians, Centennial Clinic, and McKenzie Valley Family Practice. It is the first fully accredited Patient Centered Primary Care Home in the County. It is also the first practice site in Lane County to embed behavioral health specialists.
