Advocate Health Population Health Pharmacy – Midwest Region encompasses 3 distinct pharmacy teams: the virtual team, the managed care team, and the pharmacy benefit management team. Together these teams perform many functions to optimize health outcomes, including medication therapy management, chronic disease state management, development of utilization management policies and protocols for medical benefit plans, and prior authorization reviews for select medications through pharmacy and medical benefit plans. Residents will work with all 3 pharmacy teams (the virtual team, the managed care team, and the pharmacy benefit management team) performing the functions of each team to optimize health outcomes.
