Perform pharmaceutical services related to the preparation and dispensing of medications and other pharmaceutical supplies in accordance with physicians’ orders. Identify, resolve, and prevent medication-related problems. Monitor and recommend patient-specific medication therapy. Provide drug information. Interact with patients, caregivers, other health care providers to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy. Conduct a longitudinal research project, analyze findings, and present at various professional forums.
Essential Functions
Perform all duties of a registered pharmacist within scope of licensure under supervision
Provide pharmacy services related to medication distribution, including order verification, medication preparation, dispensing, and inventory management
Adhere to safe medication use practices according to established policies and procedures
Utilize technology in the medication use process, including automation, informatics, and analytics
Participate in patient care rounds as appropriate
Perform medication therapy monitoring to identify and resolve therapeutic problems and to provide appropriate dosing recommendations
Obtain medication history and perform medication reconciliation
Document all clinical activities and interventions accurately and completely
Read and evaluate medical, pharmaceutical, and other literature to determine its scientific merit and applicability, to support pharmaceutical care and pharmacoeconomic programs
Provide accurate, adequate, and timely drug information to the health care providers and patients as requested
Interpret medical information and apply it to resolve problems or answer questions
Counsel patients and/or caregivers on anticoagulant therapy, methotrexate, and other medications as requested
Prepare written educational materials to be provided to patients and their family
Prepare periodic information such as newsletters for physicians, nurses, and pharmacists regarding medication therapy and administration
Provide in-service education programs to pharmacy staff and other health care providers
Contribute to and participate in medication use review activities as well as quality control and performance improvement activities
Enforce formulary restrictions by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting appropriate drug therapy selection
Facilitate preparation of Pharmacy and Therapeutics Committee material
Report medication errors and adverse drug reactions to organization’s incident reporting system
Investigate and document reports of adverse drug reactions
Facilitate the implementation and maintenance of safe medication use processes
Participate in Medication Safety Subcommittee
Comply with all applicable federal, state, and local laws, rules, and regulations
Ensure that the department is continuously survey ready and meets all regulatory requirements as well as accreditation standards
Attend and participate in other programs, committees, meetings, and functions as required
Stanford Health Care Tri-Valley is a comprehensive health system with medical facilities in Pleasanton, Livermore, and Dublin, California. Located in the Pleasanton campus, there is an Emergency Department along with a general acute care hospital licensed for 167 beds. Specialty services provided include cardiology, orthopedics and other surgical services.
