The Ambulatory Care PGY2 Residency at The Ohio State University College of Pharmacy strives to create pharmacists as change agents who can develop, implement, and deliver patient care services in an ambulatory setting. The program is designed to foster the development of knowledge, skills, and abilities needed to pursue and/or create an ambulatory practice in a culture of encouragement and accountability. At both primary practice sites (OSU General Internal Medicine Clinics and PrimaryOne Health), the resident is embedded in an innovative primary care setting. Both sites are at the forefront of healthcare transformation and are utilizing pharmacists to provide high-value care to patients.
Pharmacists and residents practice at the top of their license delivering care in a team-based environment using collaborative practice agreements. Residents will graduate with skills in service development, evolving reimbursement models, population health initiatives, and progressive pharmacy practice.
Practice Sites: (2 residents at General Internal Medicine, 1 resident at PrimaryOne Health)
General Internal Medicine: OSU General Internal Medicine (OSUGIM) is a network of 7 primary care patient-clinics throughout the central Ohio area and is the primary outpatient site for the nationally-recognized The Ohio State University Internal Medicine Residency program. Pharmacy residents at this site will interact with more than 100 medical residents, over 80 attending physicians, and have access to more than 70,000 patients in an interdisciplinary education-based setting across the network. Collaborative practice agreements in a variety of disease states have been established including, but not limited to, diabetes, hypertension, depression, anxiety disorders, PTSD, nicotine use disorder, weight management, and HIV PrEP. *NMS Code 632491–OSU General Internal Medicine (GIM)
PrimaryOne Health: PrimaryOne Health (P1H) is a federally qualified health center (FQHC) and is recognized as a NCQA tier 3 patient centered-medical home. P1H has ten locations throughout central Ohio, all located within culturally and economically diverse neighborhoods. P1H’s commitment to high-quality service, personal patient care, and access to state-of–the-art technology enables us to remain the fastest growing urban health center in central Ohio. Clinical Pharmacists at P1H provide patient care to the underserved patients of central Ohio as part of an interprofessional team that includes physicians, nurse practitioners, social workers/behavioral health clinicians, dieticians, and nurses. As a part of the primary care team, pharmacists and residents at P1H work with patients and providers to design therapeutic treatment plans for the management of chronic diseases such as diabetes, hypertension, tobacco use, HIV PrEP, hepatitis C, and more. For an increasing number of chronic disease states, P1H pharmacists manage conditions under collaborative practice (consult) agreements. *NMS Code 632492–Primary One Health (P1H)
Teaching:
Residents within this program will be provided a faculty appointment and will be involved in various classroom teaching activities within a recently revised curriculum at the OSU College of Pharmacy. Residents will also have the opportunity to precept IPPEs and APPEs longitudinally throughout the year. Through these experiences, the resident will obtain a teaching certificate from the College of Pharmacy.
