Residency Showcase Program Listings

This PGY2 ambulatory care residency program has been designed to foster the development of a clinician that is able to apply evidence-based medicine within various population health models or other advancing ambulatory care pharmacy settings. This will be achieved through interdisciplinary and independent learning opportunities in a variety of diverse clinical areas. Strengths of this program include longitudinal patient-centered care within interdisciplinary teams, numerous opportunities in digital medicine, population health, management, and research. A focus will be on refining skills to manage chronic disease states (with an emphasis on diabetes and hypertension), transitions of care, medication reconciliation, and care management. Elective opportunities include pharmacogenomics, tr
