The Fulton State Hospital (FSH) PGY-2 Psychiatric Pharmacy Residency Program, in affiliation with the University of Missouri Psychiatric Center and Harry S. Truman VA Hospital, offers one ASHP-accredited position for one year, beginning July 1st. The program provides the resident with the opportunity to work directly with patients suffering from treatment-resistant psychiatric disorders, including schizophrenia, bipolar disorder, depression, anxiety, personality disorders, and developmental disorders.
The residency is primarily based at Fulton State Hospital, Missouri’s only high-security forensic psychiatric facility, the largest and oldest forensic state psychiatric hospital in the state. Additional learning experiences take place at the University of Missouri Psychiatric Center for acute inpatient care, consultation-liaison and ambulatory psychiatry, and child and adolescent psychiatry, as well as at the Harry S. Truman VA Hospital for pain management.
The resident will gain comprehensive experience in a range of psychiatric pharmacy areas, including long-term and acute forensic inpatient care, outpatient care, adult acute care, child and adolescent psychiatry, consultation-liaison and ambulatory psychiatry, geriatrics, developmental disabilities, pain management, acute rehabilitation program, and pharmacy administration and leadership. The resident will participate in multidisciplinary treatment teams, daily rounds with providers, patient medication education groups, and academic teaching (including psychiatry residents, nurse practitioners, nurse practitioner students, psychology interns, physician assistant students, medical students, and pharmacy students).
The resident will also serve on committees and panels such as the Trauma-Specific Therapy Consultation Team, State Psychopharmacology Panel, FSH Psychopharmacology Panel, Incompetent to Stand Trial Committee, Research Committee, and the P&T Committee.
The program also offers a longitudinal learning experience focused on the clinical and operational management and implementation of a pharmacy-managed clozapine laboratory. Upon successful completion, the resident will meet the CLIA qualifications to serve as a director, technical consultant, or both for a non-waived laboratory performing moderate complexity testing.
By the end of the program, the resident will be prepared to practice in various psychiatric specialty settings and equipped with the competencies required for board certification in psychiatric pharmacy.

About Fulton State Hospital - PGY-2 Psychiatric Pharmacy Residency Program
Fulton State Hospital (FSH) is the oldest state psychiatric hospital west of the Mississippi River and recently underwent a dramatic transformation with the opening of a state-of-the-art facility in August 2019, revolutionizing the treatment provided to clients. FSH currently treats up to 450 clients across three buildings with two levels of security: 1) Nixon Forensic Center (NFC), a high-security center that can treat up to 300 clients; 2) the Sex Offender Rehabilitation and Treatment Services (SORTS), a high-security center that can treat up to 200 civilly committed Sexually Violent Predators; and 3) Hearnes Psychiatric Center (HPC), a minimum-security center that can treat up to 24 clients with developmental disabilities and severe mental illness. HPC also includes the Hearnes Acute Rehabilitation Program (HARP), a non-forensic acute stabilization unit that can treat up to 15 clients. HARP provides short-term stabilization and skills development for individuals with developmental disabilities who are experiencing acute psychiatric symptoms, helping them prepare for a successful return to the community once they are ready for discharge. FSH provides care to a diverse population,... with clients from rural and urban areas across Missouri, and a demographic breakdown of approximately 60% Caucasian, 37% African American, and 3% representing Native American, Hispanic, and other backgrounds. Nearly 90% of clients are male, and the majority fall between the ages of 22 and 60. FSH also serves individuals from various sexual identities and orientations, including clients who are deaf, hard of hearing, or speak English as a second language, with interpreting and translation services provided through the Interpreting Services Department. The legal status of most clients at FSH is determined by the courts, with the majority referred as incompetent to stand trial (IST) or as not guilty by reason of mental disease or defect (NGRI). Some clients are eventually found permanently incompetent to stand trial (PIST) and remain at FSH for treatment. Around 19% of clients are initially committed as IST, with most being restored to competency within 5-10 months. Additionally, 28% of clients are PIST, 15% are NGRI, 25% are civilly committed as SVP, and 12% are admitted voluntarily by guardian (VBG). The length of stay depends on several factors, including legal status, recovery progress, mental illness severity, and space availability at receiving facilities. FSH provides evidence-based treatment through long-term rehabilitation programs once a client’s legal status is determined as SVP, PIST, NGRI, or VBG. Treatment programs include the Social Learning Program (SLP), New Outlook Program for Behavior and Mood Self-Management (NOP), Recovery and Self-Motivation (RSM), and SORTS. Clients present with a wide variety of clinical diagnoses, covering nearly all major DSM-5 categories, with the most common being schizophrenia-spectrum disorders, mood disorders, intellectual disabilities, personality disorders, and paraphilic disorders. FSH also treats a smaller population with neurocognitive disorders.
