Purpose Statement: SSM Health St. Agnes Hospital’s PGY1 Pharmacy Practice Residency was established in 2021. The program prepares residents to fill positions as clinical providers of advanced patient care services in a community hospital pharmacy setting and/or pursue PGY2 training. The program will equip the resident to provide direct patient care, engage in collaborations with physicians and other healthcare providers, educate pharmacy students and other members of the health-care team, and provide leadership for advancing hospital practice and the profession.
Program-Specific overview: Pharmacists at St. Agnes Hospital provide decentralized services in the emergency department and on the medical/surgical, intensive care, cardiology, and neurology units. Pharmacist services include admission medication reviews, rounding with providers, glycemic management services, antimicrobial stewardship, kinetics services, discharge medication reconciliation, and patient education. Pharmacists are also located in the cancer center, anticoagulation clinic/infusion clinic, and ambulatory clinic.
Application Requirements
1. Candidates must be a graduate of an ACPE-accredited College of Pharmacy and eligible for licensure in the state(s) for which they will practice. All applicants must use the Pharmacy Online Residency Centralized Application Service (PhORCAS).
