About Our Ambulatory Care Program:
The purpose of this residency is to prepare the trainee with the necessary skills to excel as an Ambulatory Care Pharmacy Specialist. At this ASHP-accredited residency program, the resident will have opportunities to learn in a variety of practice environments, including a pharmacist-managed pharmacotherapy clinics, family medicine / internal medicine clinics, cardiology clinics, anticoagulation clinics, telehealth medicine, and population health management. The resident will gain experience with chronic disease management under a collaborative practice agreement for commonly encountered disease states such as diabetes, hypertension, heart failure, asthma/COPD, smoking cessation, and anticoagulation. The resident will also be actively involved with educational and medication management presentations to patients, providers, and staff in ambulatory clinics. Additionally, the resident will have opportunities to assist with the training and education of pharmacy students and PGY1 Pharmacy Residents. The resident will also be enrolled in our Resident Development Series designed to help residents excel personally and professionally. The resident will have a mentor to support them throughout the year and will have the opportunity to participate in the system-wide resident wellness series. Requirements:
The resident will be required to be licensed in Indiana by August 1st and strongly encouraged to obtain licensure in Illinois by December 31st of that residency year to participate in Illinois-based elective rotation opportunities.
Resident candidates must participate and adhere to the rules of the Residency Match Program and submit all materials through PhORCAS. The below items are required for a complete application before the application deadline of January 2nd.
