AdventHealth East Orlando is a not-for-profit 295-bed academic, community hospital located in Orlando, Florida. We have two offsite Emergency Departments at Waterford Lakes and Lake Nona, and our 80-bed Emergency Department is one of the busiest in central Florida. We are home to two pharmacy residents, three separate medical residency programs (Podiatry, Family Medicine, and Emergency Medicine), and a state-of-the-art Medical Simulation Lab. We are a clinical teaching site for several colleges of pharmacy, medicine, and nursing. Our ambulatory care pharmacists work in our Transitions of Care Clinic to manage patients with complex disease states and assist with bridging the gap between hospital discharge and community providers. Through structured medication management, pharmacists work to enhance a patient’s understanding of their health care needs in addition to preventing further hospital admissions.
Our pharmacists provide direct patient care with the multidisciplinary team in a wide variety of decentralized specialties, including Internal Medicine, Emergency Medicine, Antimicrobial Stewardship, Oncology, Ambulatory Care, Critical Care, and Administration. Other clinical services offered include medication reconciliation at time of admission and discharge, antimicrobial stewardship, glycemic management, vancomycin dosing and monitoring, aminoglycoside dosing and monitoring, metabolic support for patients requiring TPN, renal dose adjustments, and IV to PO de-escalations. We have evidence-based bedside rounding services in the ICU and with our Family Medicine residents, in addition to multidisciplinary rounds on other floors aimed at decreasing length of stay and readmissions. Being part of a 9-hospital system, there is opportunity for involvement in division level learning experiences and committees, including Clinical Pharmacy, Emergency Medicine, Pharmacy and Therapeutics, Medication-Safety and Antimicrobial Stewardship.
