Pharmacists at the Forefront of Geriatric Care

The PGY2 Residency in Geriatrics offers patient care experiences in a variety of team-based settings. Residents will learn to work collaboratively with primary care teams, acute care providers, community partners and patients themselves to initiate and adjust medication therapy. In addition to participating in chronic disease state management, they will be trained to recognize and intervene appropriately on geriatric-specific syndromes such as polypharmacy, fall risk and dementia and delirium. The resident will also participate in research, education, committee work and practice leadership, and stewardship activities.

Program Strengths

• Focus on geriatric ambulatory care across outpatient settings (in-home and community-based residential care)
• Work within multiple models of providing geriatric care (including the Program for All-Inclusive Care of the Elderly and home-based primary care)
• Longitudinal clinical experiences provide continuity and allow the resident to follow patients as their preferences and care needs evolve.
• Interdisciplinary care models allow residents both to serve as a resource AND to learn from other health care professionals.
• Be part of the Age Friendly Health System and geriatric mini-fellowship for primary care physicians

By the end of the residency program, the resident should be able to:

• Establish a collaborative, interdisciplinary practice to provide evidence-based, patient-centered treatment; serve as an authoritative resource for the best use of medications
• Make patient-specific pharmacotherapy recommendations that are compatible with an older adult’s medical needs, goals of care, values and preferences
• Demonstrate leadership and practice management skills
• Demonstrate excellence in training and education

Residency Program Purpose Statement

The purpose of the PGY2 Geriatric Pharmacy Residency program at Providence St. Joseph Health Oregon Region is to prepare geriatric pharmacist specialists to provide direct patient care as members of a collaborative interdisciplinary practice.

Residency Structure

Residents will engage in direct patient care rotations with Providence ElderPlace (PACE program), Providence Medical Group at Home (home-based primary care) and the Geriatric Assessment Clinic and Fall Assessment Clinic at Providence Medical Group resident clinics. They will have longitudinal learning experiences in Evidence-Based Medicine, Health Education, Leadership and Administration and Medication Safety, and will complete a major project. Time will be allocated for the resident to complete one or more elective learning experiences per their practice interest(s).
