SSM-Health is a Catholic, not-for-profit health system serving the comprehensive health needs of communities across the Midwest through a robust and fully integrated health care delivery system. SSM Health Monroe Hospital is a 225,000 square-foot facility offering a nature-inspired design conducive to health and healing that opened in 2012.  With a 58-bed operating capacity, our hospital services the medical, surgical, intensive care, emergency and obstetrical needs of patient in our region.  SSM Health Monroe Clinic Medical Group is a multi-physician practice with over 120 employed providers offering a broad range of health care services across 12 clinic locations. The main campus located in Monroe, Wisconsin opened a building expansion in 2019 and features an innovative Integrative Care Team functionality.  The residency program's primary focus is Ambulatory Care, with a strong emphasis on teaching. Required rotations include Pharmacotherapy Clinic, Community Pharmacy Practice - Outpatient Medication Management, Hospital Medicine, Medication Use & Drug Policy, and Pharmacotherapy Lab Teaching at the University of Wisconsin School of Pharmacy.  Through a comprehensive scope of practice, a variety of patients are managed within the Pharmacotherapy Clinic. These include patients with diabetes, hyperlipidemia, hypertension, heart failure, anticoagulation needs, pharmacogenomic counseling needs, pain management, tobacco use, thyroid issues, depression/anxiety, respiratory disorders, and more.  Elective rotations include Pulmonology, Nephrology, Neurology, Behavioral Health, Primary Care, Emergency Medicine, Antibiotic Stewardship, hospital, and Oncology. Research opportunities focus on patient outcomes in an ambulatory setting.  There are multiple opportunities to collaborate with the Family Medicine Residency Program.  There is a required Public Health Outreach Project and a Professional Development longitudinal rotation, as well as an annual project requirement. 

Applicants must be a graduate from an American Council of Pharmaceutical Education accredited School of Pharmacy with a PharmD degree and eligible for licensure in the State of Wisconsin.
