SSM Health Dean Medical Group encompasses over twenty primary care clinics across the greater Madison area. Since its inception in 2015, our clinical pharmacy team has grown into a robust and dynamic presence within the organization. Pharmacists are embedded in six major clinics and extend their expertise remotely to additional sites, ensuring consistent, high-quality care across the region.
Operating under collaborative practice agreements, our pharmacists are integral members of the healthcare team, providing autonomous, comprehensive medication management for chronic conditions. We are committed to value-based care, with a strong focus on improving population health—especially among underserved communities.

Our program offers a well-rounded ambulatory care experience, fostering collaboration with interdisciplinary medical teams and serving a diverse patient population across numerous disease states.
Completion of a PGY1 residency, eligible for licensure in WI, and eligible to obtain a NPI.

Our network consists of more than 60 clinics in south-central Wisconsin, SSM Health Davis Duehr Dean Eye Care, insurance provider Dean Health Plan, and pharmacy benefits company Navitus Health Solutions. Approximately 500 physicians provide primary, specialty and tertiary care in the clinics. The system serves more than 400,000 health plan members.
SSM Health Dean Medical Group joined the SSM Health system in September 2013. SSM Health is headquartered in St. Louis and owns several hospitals nationwide, including SSM Health St. Mary’s Hospital - Madison, SSM Health St. Mary’s Hospital - Janesville and SSM Health St. Clare Hospital - Baraboo.
