The mission of Baylor St. Luke's Medical Center's (BSLMC) Department of Pharmacy is to "embody clinical and operational excellence as we surpass boundaries of pharmacy best-practice across the continuum of care for the wellness of our patients and community". Our vision is to "be a recognized leader in the advancement of pharmacy best practice with a world-class customer service team".In 2010, the Department of Pharmacy in collaboration with the University of Houston College of Pharmacy began to realize our vision by receiving the American Society of Health-System Pharmacists Best Practices Award in Health-System Pharmacy for our Center for Antimicrobial Stewardship and Epidemiology (CASE). The CASE program was recognized for improving patient care through clinical service, teaching and research. In 2012, we received the Texas Society of Health-System Pharmacy's Residency Excellence Award for our PGY1 Pharmacy Residency Program. The evolving pharmacy practice model at Baylor St. Luke’s leverages industry leading technology to optimize safety and efficiency in the medication use process. Our expert staff of pharmacists and technicians partner with physicians and nurses to provide optimal care to our patients. The Clinical Pharmacists bring specialized expertise to various service lines in the hospital, support the on-going education and development of the pharmacy staff, precept pharmacy students and pharmacy residents. The Clinical Pharmacist staff oversees and advises the clinical care teams on all steps of the medication use process and provides direct patient care throughout the hospital. The Certified Pharmacy Technicians prepare sterile products, manage first doses and unit-based cabinet refills from the Medcarousel, barcode and package unit dose medications, enter medication orders into the pharmacy information system, check other technicians work and provide support throughout the department.Baylor St. Luke’s Medical Center is a leader in technology adoption to advance patient safety, including barcode medication administration, computerized physician order entry and an integrated electronic health record. The pharmacists have easy access to patient information and utilize trigger reports to target high risk therapeutic situations. Medication distribution is provided from an automated central pharmacy, unit-based dispensing cabinets, centralized USP 800 compliant cleanroom, and satellite pharmacies in the peri-operative and emergency department locations.Pharmacy residencies and fellowships at Baylor St. Luke’s Medical Center include:

The applicant must be a highly motivated individual who desires to obtain advanced education and training leading to an enhanced level of professional practice. Each applicant will receive a copy of this Residency Manual via email during the application process.
The applicant must be enrolled in (or be a graduate of) an ACPE-accredited advanced pharmacy degree program or have equivalent experience.
Applicants must register with the NRMP and must adhere to the rules of the Match. Registration with the NRMP will be waived only if a residency position remains unfilled after Phase I and Phase II Match.
Applicants to the PGY2 residency programs or fellowships must be completing or have completed an ASHP-accredited PGY1 pharmacy residency.
The applicant should have prior hospital pharmacy experience.
The application deadline will vary slightly each year but will generally occur during the last week of December or first week of January. Applications may be accepted after this date at the hospital's discretion only if the interviews offered for the current residency recruitment year are not full.
The applicant must submit to PhORCAS the following information by the specified deadline:
Completed PhORCAS Application. Applications will only be accepted through the Pharmacy Online Residency Centralized Application Service, or PhORCAS. This service may be accessed by visiting www.ashp.org/phorcas. Applicants are responsible for completing all required materials and verifying availability of references by the posted deadline.
Official school of pharmacy and/or graduate school transcripts
Curriculum vitae which should include all rotational experiences (completed and pending ones)
Three letters of professional references. The reference writers must complete the online information via PhORCAS. The reference writers will need to address the applicant’s potential for success in the residency program. References should be from either faculty members in the applicant’s professional degree program or from practicing pharmacists with whom the applicant has had work or patient care experience.
Letter of intent
A professional photo (e.g. passport photo) of the candidate must be submitted after the applicant has received and accepted the offer to interview.
All rules and regulations of the ASHP residency matching program will be strictly followed.
For the PGY1/PGY2 Health-System Pharmacy Administration residency, the applicant must also apply to the University of Houston College of Pharmacy. Prior to on-boarding, the selected candidate must:
Be a registered pharmacist with the Texas State Board of Pharmacy and in good standing
Have completed an ACPE approved 20 hours sterile products course for aseptic technique
