Residency Showcase Program Listings

As a PGY1 Community-Based Pharmacy Resident at UH Meds, you will play a crucial role in supporting patients in achieving their health objectives through personalized medication management and population health initiatives. Collaborating within the primary care setting, you will work alongside a diverse team of healthcare professionals to develop comprehensive care plans tailored to each patient's needs. Your responsibilities will include ensuring seamless access to medications for patients as well as actively managing drug therapy. You will also practice in a select group of specialized teams including our Cardiology clinics and Culture Callback teams. In addition, you will assist in enhancing transitions of care through initiatives like the Meds to Beds program, discharge counseling, and routine medication dispensing across the University Hospitals system. Finally, we have a number of elective experiences to choose from including Endocrinology, Women's Health, Employee Health, Centralized Refill Authorization, and Advanced Primary Care. 

Throughout your residency, you will also have the opportunity to engage in patient education, medication adherence strategies, and preventive care initiatives, all aimed at fostering a culture of wellness and patient empowerment. Beyond the clinic, you will also improve your skills in research and project management. Our Research Committee provides a strong backbone to our residents by offering guidance and oversight while still encouraging independent development and management of major and minor projects.

This residency program will provide you with a solid foundation in community-based pharmacy practice, equipping you with the necessary skills to excel in a dynamic healthcare environment. You will have the chance to contribute to improving the quality of patient care, participate in interdisciplinary projects, and engage in ongoing professional development to enhance your competency as a clinical pharmacist.
