University of Kentucky College of Pharmacy PGY1 Community-Based Community Residency Program
This is a multi-site residency with one position in each of the following sites: PrimaryPlus Morehead, Kroger Pharmacy - Lexington, and St. Matthews Community/Specialty Pharmacy. PrimaryPlus is a group of Federally Qualified Health Centers (FQHC) serving northeastern Kentucky and southeastern Ohio. The goal of PrimaryPlus is to provide advanced, affordable healthcare and to connect as many pieces of the healthcare puzzle as possible. The resident’s primary practice site is at the pharmacy in the PrimaryPlus Clinic in Morehead, KY. Kroger Pharmacy is a supermarket chain pharmacy that has over 100 pharmacies in five districts in the Louisville Division. The Kroger Lexington site is located in Lexington, KY providing MTM, immunizations, and employee wellness/coaching programs. Additionally, the Lexington site has an American Diabetes Association-recognized program delivering diabetes education and prevention programs. The St. Matthews Community/Specialty Pharmacy is a unique practice located in Louisville KY that performs a variety of clinical services across multiple disease states in both community and specialty pharmacy. Examples include comprehensive medication management, substance use disorder, and transitions of care. The University of Kentucky College of Pharmacy assists the resident with practice-based project and teaching experiences.
University of Kentucky College of Pharmacy PGY2 Ambulatory Care Residency/Bluegrass Community Health Center
This postgraduate year two (PGY2) Ambulatory Care Pharmacy Residency Program is jointly-sponsored by Bluegrass Community Health Center and the University of Kentucky (UK) College of Pharmacy. It is an established pharmacy residency program at Bluegrass Community Health Center, a Federally Qualified Health Center and National Committee for Quality Assurance Level 3 Patient-Centered Medical Home. This program is intended to foster growth of residents from a generalized practice to an elevated practice focused on the care of patients in ambulatory care settings. The purpose of the program is to develop clinical pharmacists with expertise in the area of medication therapy management, medication review, pharmacotherapy consultation, collaborative drug therapy management, transitions of care, medication education, medication assisted treatment, and other related services.
University of Kentucky College of Pharmacy PGY1 Community-Based Community Residency Program
Graduate of accredited school or college of pharmacy. Licensed or eligible for licensure in Kentucky for 144515 (Kroger Pharmacy Lexington), 144522 (PrimaryPlus Morehead), and 144521 (St. Matthews Community/Specialty Pharmacy). In addition to PhORCAS requirements for application, the program requires the following: a) Describe a significant current event (e.g., an advocacy initiative, new legislation, large collaborative efforts involving health initiatives or partnerships, etc.) at the local, state, or national level that impacted or has the ability to impact the advancement of pharmacy practice; and b) References should be provided by an employer, a preceptor, and a faculty member. If the applicant cannot obtain references from an employer, please choose another preceptor from an ambulatory/community Advanced Pharmacy Practice Experience.

Doctor of pharmacy degree; completion of a PGY1 pharmacy practice residency or general clinical residency; application, CV, transcripts, & letters of recommendation (via PhORCAS); eligibility for licensure in Kentucky. The University of Kentucky College of Pharmacy Community Pharmacy Residency Program (CPRP), established in 2001, is dedicated to developing community practitioners who are providers of direct patient care services and innovators in the advancement of community pharmacy practice.
