								<description>Vancouver, Washington,  PeaceHealth Southwest Medical Center (PHSW)&#xa0;began offering a one-year ASHP-accredited residency in pharmacy practice in July of 1996. When the program began, two residents were selected to participate in the program. After successful completion of the third resident class, the program expanded to the selection of three residents in 1999, four residents in 2007. In 2017, PHSW joined with PeaceHealth St John to offer six residency positions between the two hospital sites. Starting in 2019, each hospital offered&#xa0;their own residency program and PHSW&#xa0;resumed&#xa0;training four residents.&#xa0; PHSW has continued to expand our residency program with the addition of a PGY2 Ambulatory Care Pharmacy residency position.&#xa0;&#xa0;While&#xa0;structured to meet the intent of the current ASHP Post-graduate year one (PGY1) residency program standards, training experiences are individualized based on existing resident competencies and personal interests. 
 The purpose of this residency program is to develop competent clinical practitioners who have accelerated their growth beyond entry-level professional competence in patient care and in practice management, and have further developed leadership skills that may be applied in any direct patient care position in a variety of practice settings upon completion of the residency. 								<description>Seattle, Washington,  PGY1 pharmacy residency programs build on Doctor of Pharmacy (PharmD) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. 
 Pharmacists completing this residency will be competent and confident practitioners in multiple health care environments and across the continuum of patient-centered care in an integrated health-system. Program graduates will be prepared to enter clinical practice. These pharmacists will exercise skill in educating other health care professionals and patients on medication related topics. The residents will develop their presentation and teaching/preceptor skills. They will perform needs assessment and display problem solving skills for developing and evaluating programs.  St. Anne Hospital Residency Program; Burien, WA 
 The St. Anne Hospital pharmacy residency is a PGY-1 program fully accredited by the American Society of Health-System Pharmacists (ASHP). Our mission is to develop competent, confident pharmacy practitioners who will function effectively in various health care environments, while developing skills in pharmacy practice management. Clinical learning experiences assist the residents to expand their knowledge of therapeutics and aid in the development of critical thinking and problem-solving skills. Leadership skills and professionalism will be strengthened through the practice management component of the residency program. 
 The residency experience is tailored by the resident development plan to help the resident achieve individual career goals. A variety of opportunities will be afforded to the resident in direct patient care, precepting, education and management experiences. This training is evidence-based and allows the resident to become an integral member of an interdisciplinary team. 
 Upon completion of the program, the resident will be able to provide direct patient-centered care in acute care settings. The resident will be prepared to function in the role of a clinical pharmacist or pursue additional training in a PGY-2 program, such as critical care, emergency medicine, infectious diseases, and management.

The resident will be encouraged to submit the project for publication and present at state or national pharmacy meetings. 
 
 
 Required longitudinal experiences - continuous 
 Longitudinal experiences will occur throughout the residency year and encompass activities such as performing Drug Utilization Evaluations, presenting recommendations to Pharmacy and Therapeutics Committees, student precepting, and creating a monthly newsletter directed to all&#xa0;Virginia Mason Franciscan Health clinical staff. Residents will also be directly involved in the education of providers, pharmacist interns and pharmacy technicians.

 St. Joseph Medical Center PGY-1 in an Ambulatory Setting 
 
 
 The St. Joseph Medical Center PGY1 residency program in an ambulatory setting is an ASHP candidate status program that began July 2020 with two residents and is scheduled for a credentialing survey in March 2021. This residency program is an expansion into the ambulatory care arena to match our developed ambulatory care services including over 25 pharmacists practicing in primary care, heart failure, rheumatology, population health, oncology, anticoagulation and hospice. Since 1985 St. Joseph Medical Center has provided an excellent PGY1 residency program experience in an acute care setting. We strive to develop a foundation of critical thinking and leadership skills that will promote continuous professional development and innovation in healthcare. Our program fosters a skill set to prepare residents for practice as clinical pharmacists in a variety of patient care, academic, and research settings, including PGY2 residency and fellowship experiences. The residency recruits for two resident positions from candidates throughout the United States. 
 
 
 The residency program is focused on providing diverse opportunities to develop proficient clinicians in provider clinics, pharmacist-run clinics, and population health arenas. Additionally, the residency provides experiences to intricately understand the patient&#8217;s continuum of care from inpatient to ambulatory to hospice settings. During the program, the resident will establish and manage a yearlong project geared towards creating or optimizing pharmacy services provided in the ambulatory care setting. As integral members of the health care team, residents work to achieve optimal drug therapy outcomes for their patients. They will display leadership and professionalism in all aspects of their practice and function as effective communicators and educators. 
 
 
 Our residency program, based in a non-academic health care setting, has become a catalyst for innovative pharmacy practice models within Virginia Mason Franciscan Health. Rotation areas are designed to provide the resident with a well-rounded clinical and administrative experience. Within each rotation, additional time may be devoted to areas of interest, thus providing the opportunity for specialized training. Clinical rotations are designed to enhance the resident&#8217;s expertise in promoting, assessing and ensuring safe, efficacious and rational drug therapy. 
 
 About the Director: St. Joseph Medical Center PGY-1 in an Ambulatory Setting Residency 
 
 
 Kristin Peterson, PharmD, BCACP 
 Kristin received her Bachelor of Science in Pharmacy in 1997 and her Doctor of Pharmacy Degree in 1998, both from the University of Washington. She also completed a Pharmacy Practice Residency at the Veteran&#8217;s Administration in Seattle in 1998. She has worked for Virginia Mason Franciscan Health&#8217;s St. Joseph Medical Center since 1998 and currently is the PGY-1 in an Ambulatory Setting Residency Program Director and the Ambulatory Clinical Pharmacy Supervisor for the Ambulatory team. Kristin has played an integral role in the development of all Ambulatory Pharmacy Services. She is a Board Certified Specialist in Ambulatory Care Pharmacy.

 Residents will be assigned to an Anticoagulation Clinic and Hospice Pharmacy within the health system for further experiential development through staffing opportunities. 
 
 
 Stipend and benefits 
 Benefits include 30 days of Paid Time Off (PTO), medical, dental and life insurance for residents and dependents. Paid educational leave and a travel allowance are available to support attendance at state and national meetings. The current stipend is $58,947 annually/$28.34 per hour. 
 
 
 Residency project - continuous 
 Residents conduct a project aimed at identifying, organizing and solving a clinical or management related problem that aligns with the organizations&#8217; strategic initiatives and focus. This often leads to protocol/guideline development or new roles for pharmacists. This project will be presented at the Western States Conference for Pharmacy Residents, Fellows and Preceptors. 								<description>Silverdale, Washington,  St. Michael Medical Center is a new PGY1 residency program in Candidate status with ASHP. When the first residents started in 2020, the program submitted an Application for Accreditation (candidate status). The on-site accreditation survey is planned for Spring 2021. Once accreditation is gained, it becomes retroactive to the date the application was filed with ASHP, and the residency program will then provide residents with a certificate indicating completion of an ASHP-accredited residency. As part of a new residency program, residents will have the unique opportunity to actively develop many aspects of the residency. The program is recruiting for two residency positions. 
 
 
 Pharmacists completing this program will function as independent, patient-focused practitioners who are able to provide care in multiple environments across the pharmacy practice continuum.  About the Residency Program Director 
 Andrew Harbison is currently a Critical Care Pharmacist at Virginia Mason Franciscan Health St. Michael Medical Center. He graduated from Washington State University with his Doctor of Pharmacy degree and completed a pharmacy residency at Providence Sacred Heart Medical Center in Spokane, Washington. Shortly after, he moved to the Puget Sound area and has worked at St. Michael Medical Center since 2011. He is board certified in Pharmacotherapy.

Clinical rotations are designed to enhance the resident&#8217;s expertise in promoting, assessing and assuring safe, efficacious and rational drug therapy. 
 
 About the Director: St. Joseph Pharmacy Residency 
 
 
 Ferowsi Pecoraro &#xa0;is the Residency Program Director for the PGY-1 pharmacy residency at Virginia Mason Franciscan Health St. Joseph Medical Center. She is also one of the regional Oncology Pharmacy Supervisors overseeing pharmacy services for oncology and five hospital-based infusion clinics. Ferowsi graduated from Washington State University with her Doctor of Pharmacy degree and completed a Pharmacy Practice Residency at Good Samaritan Hospital in Puyallup, Washington. Shortly after, she moved to New York and worked at Mount Sinai Hospital as an oncology pharmacist. Ferowsi returned to Washington and in 2006 became the Oncology Pharmacist Specialist at St. Joseph Medical Center where she helped to grow clinical pharmacy services to support the oncology service line as well as expand the oncology pharmacist&#8217;s role. Ferowsi has always been passionate about her role as a preceptor and educator. She has been a guest lecturer for oncology pharmacy courses at Washington State University and Pacific University. She is Board Certified in Oncology and has practiced in this area for the past 14 years sharing her knowledge with pharmacy residents and students.

 Required Rotations&#8212;40 weeks Residents participate in eight required rotations spanning 40 weeks, with the opportunity for further experiences based on individual background and interests. All of these rotations are direct patient care rotations with the exception of Practice Management. Critical Care 8 weeks Emergency Medicine 6 weeks NICU 2 weeks Internal Medicine 6 weeks Oncology 6 weeks Practice Management 4 weeks Ambulatory Care 4 weeks Infectious Disease 4 weeks Optional Rotations&#8212;6 weeks Optional rotations are direct patient care specialty rotations, some of which may be completed off- site. Additionally, residents have the opportunity to propose and create alternate rotations based on clinical interests. Hospice 2 weeks NICU 2 weeks Advanced Critical Care 2-6 weeks Advanced ED 2-4 weeks Advanced Int Med 2-4 weeks Advanced Oncology 2-6 weeks 
 Orientation &#38; Concentrated Learning&#8212;3 weeks During orientation, residents are expected to acquire the following general skills necessary to care for patients in required rotations and longitudinal experiences. The following educational experiences will occur during this orientation time: &#8211; Anticoagulation Management &#8211; Aminoglycoside Dosing &#8211; Basic Oncology &#8211; Basic Skills &#8211; Code Blue &#8211; EPIC Training &#8211; Pain Management &#8211; Renal Dosing Adjustment &#8211; TPN &#8211; Vancomycin Dosing. 
 Residency Project&#8212;Continuous Residents conduct a project aimed at identifying, organizing and solving a clinical or management-related problem, which often leads to the development of a new pharmacy service. This project will be presented at the Western States Conference for Pharmacy Residents, Fellows and Preceptors. The resident will be encouraged to submit the project for publication and present at state or national pharmacy meetings. Required Longitudinal Experiences&#8212;Continuous Longitudinal experiences will occur throughout the residency year and encompass activities such as performing Drug Utilization Evaluations, presenting recommendations to Pharmacy &#38; Therapeutics Committees, student precepting/PEARLS and creating a monthly newsletter directed to all CHI Franciscan Health clinical staff. Residents will also be directly involved in the education of providers, pharmacist interns and pharmacy technicians. Residents will be assigned to a hospital within the health system for further experiential development through staffing opportunities.</description>

 The Pharmacy Residency Program is a 12-month program with four (4) resident positions available annually. 
 
 This program enables residents to participate in evidence-based, patient-centered medication management. 
 Leadership, writing, and teaching skills are developed through clinical rotations as a member of the multidisciplinary team and by participation in administrative and operational services. 
 Competency is shaped by feedback from preceptors and from self-evaluation. Each resident&#8217;s learning plan is individualized. 
 Residents are eligible for full benefits. 
 Washington pharmacist licensure is required (must be completed by September 1st). 
 A major clinical or administrative project must be completed during the course of the residency year. Project selection is a joint effort between the resident, program director, and selected pharmacist project advisors. 
 Residents are required to staff 3 shifts per month. Residents may pick up additional shifts at full pharmacist wages.
