At Schar, they will staff, counsel patients, educate pharmacy and other team members about new medications and therapies, develop specific patient care programs and disease state management programs based on current need and the resident&#39;s interests. The resident will also monitor and manage patients in our Patient Assistance Program oral chemotherapy program. Numerous &#xa0;opportunities exist for the resident to gain management experience and to initiate quality improvement projects and new business projects working with the pharmacy managers and the Director of Retail and Specialty Pharmacy. The resident will also staff at the Inova Fairfax Heart and Vascular Institute Pharmacy Plus location. A Diabetes Management rotation is available at our Fair Oaks Hospital Pharmacy Plus location. The resident will also have the opportunity to manage ambulatory patients with a variety of chronic disease states, with a focus on diabetes and HIV.&#xa0;

 The PGY2 Ambulatory Care residency program at the Captain James A. Lovell Federal Health Care Center (FHCC) is designed to produce advanced pharmacy practitioners in primary care. The resident is offered the opportunity to obtain a teaching certificate through the Rosalind Franklin University College of Pharmacy, precept pharmacy students, and participate in didactic teaching. This PGY2 residency program will guide the resident in developing accountability, practice patterns, habits, and expert knowledge, skills, attitudes and abilities in Ambulatory Care Pharmacy. Building upon the broad-based competencies achieved in a PGY1 residency, the resident will achieve the ability to provide care in the most complex of cases or in the support of care through practice leadership. Upon completion of this residency, the resident will have the ability to function independently as a practitioner by conceptualizing and integrating accumulated experience and knowledge and transforming both into improved medication therapy for patients. A resident who successfully completes this PGY2 residency should possess competencies that enable attainment of board certification in ambulatory care pharmacy (BCACP). &#xa0;Requirements:

								<description>Valparaiso and Mishawaka, Indiana,  The HealthLinc Pharmacy Residency Program is a PGY1 Community Pharmacy Residency at HealthLinc Community Health Center, with a heavy focus on ambulatory care for underserved populations. Residents have the opportunity to provide chronic disease state management, by working under more than 10 collaborative practice agreements: including specialty disease states such as hepatitis C virus and complex diabetes patients on pump therapy. The medication distribution learning experience is completed in the  on-site entity owned and operated community pharmacy. The residency also partners with Purdue University for the research, business plan, and drug information learning experiences and will complete a teaching certificate through the Indiana Pharmacy Teaching Certificate Program (IPTeC).

 Purpose 
 PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification and eligible for postgraduate year two (PGY2) pharmacy residency training&#xa0; 
 Pharmacy Services 
 Coastal Medical has a unique and innovative pharmaceutical care program with over 10 clinical pharmacists working in an Accountable Care Organization. Clinical pharmacy services are provided to all of the adult internal medicine practices, family medicine practices and the sub-specialty practices including cardiology and pulmonology.&#xa0;Pharmacists are a part of Coastal Medical&#8217;s population health strategy and integral members of our care management programs. Pharmacists&#xa0;work under a collaborative practice agreement with all of the Coastal Medical providers.&#xa0; The collaborative practice agreement scope of practice includes diabetes, cardiology, respiratory diseases, smoking cessation, HIV/AIDS and general medication therapy management.&#xa0;In addition, the team provides opioid and antibiotic stewardship services, conducts comprehensive medication reviews for targeted patients via OutcomesMTM, and provides drug information services to the providers. Members of the team also participate in multidisciplinary care conferences. Patients may be referred to telephonic or face to face services by any member of the healthcare team or patients may self-refer.&#xa0; Consultations are offered to patients at no charge.&#xa0;The pharmacists document all patient interactions in the electronic health record which is shared and viewable by each member of the healthcare team.&#xa0; 
 In the pharmacy program, pharmacists have several unique roles including office-based pharmacists,&#xa0;population health management pharmacists,&#xa0;or managed care pharmacist. Office-based pharmacists are assigned to an office depending on the day of the week to provide clinical pharmacy services, opioid stewardship, and antibiotic stewardship. Population health management pharmacists provide services to patients in the diabetes management program, Coastal@Home program, heart failure program, and chronic obstructive pulmonary disease (COPD) program.&#xa0;The managed care pharmacist works with payers to meet all medication related quality measures and outcomes.&#xa0;The managed care pharmacist also supervises the pharmacy technician team and the medication refill protocol. All pharmacists also participate in the education and training of advanced pharmacy practice experience (APPE) students from the University of Rhode Island.&#xa0; 
 Program Overview 
 The Coastal Medical/URI residency is a unique 12-month PGY-1 residency providing direct patient care in outpatient primary care practices. The resident works collaboratively as a member of the healthcare team to prevent and alleviate medication-related problems, optimize patient outcomes, advance the workflows for medication use and safety in primary care, and evaluate the impact of medication management services. The resident will receive training in the delivery of direct patient care, the enhancement of professional communication and interpersonal skills, academic teaching, student precepting, and management skills. Clinical experiences may include adult internal medicine, diabetes, cardiology, pulmonology, smoking cessation and pain management. The resident will have the opportunity to complete a medication class review and provide drug information services while on rotation at the URI Drug Information Services. 
 Our student precepting environment allows residents to develop strong teaching skills. Our capable research staff is an excellent resource for assisting the resident in developing a solid foundation in research design and analysis.&#xa0;

 The resident will meet quarterly with the residency program director to tailor the resident&#8217;s schedule to meet individual goals. Upon completion of the program, the resident will be awarded an ASHP accredited residency certificate if all program requirements are met. 
 The resident will be required to attain a Rhode Island professional pharmacy license within 90 days of the start date of the residency. Failure to attain this license can result in the termination of employment with the residency program. 
 Program Benefits 
 The resident will receive a 1-year stipend in the total amount of $43,500. The resident is eligible for Coastal Medical health insurance coverage. 10 vacation days and 5 sick days are available. A travel reimbursement will be provided to attend a clinical meeting to present the resident&#8217;s research project. The resident is also provided with a dedicated workspace and a laptop computer.

								<description>Brooklyn, New York,  The One Brooklyn Health&#39;s PGY2 Ambulatory Care Residency is an organized, directed, postgraduate training program in ambulatory care pharmacy practice. The goal of this residency program is to enable residents to develop the knowledge and experience necessary to provide pharmaceutical care to ambulatory patients with a strong emphasis on developing and mastering clinical skills in the outpatient setting. 
 This program will offer the resident the opportunity for longitudinal patient care experiences within our Medication Management, Anticoagulation, Psychiatry, &#xa0;and HIV Clinics.&#xa0; In addition, the resident will have the opportunity to rotate through specialty clinics as part of elective learning experiences. The resident will have opportunities to conduct direct patient visits, participate in community outreach, and collaborate with physicians, nurses, social workers, and other clinical staff.
