 Family Pharmaceutical Services is a small chain of community pharmacies owned by Kaleida Health. The main practice location for this residency is High Street Prescription Center, which operates inside Buffalo General Medical Center. BGMC is our health system&#8217;s largest teaching hospital.&#xa0; Our outpatient pharmacy supports inpatient hospital staff through the transitions of care process.&#xa0; We also support the many outpatient clinics as well as our home health nursing agency.

								<description>Columbus, Ohio,  The Nationwide Children&#8217;s PGY1 Community-Based Pharmacy Residency focuses on training pharmacists in disease state management in a community and ambulatory care setting. 
 Program Overview 
 The Department of Pharmacy at Nationwide Children&#39;s Hospital continues to expand the services offered in ambulatory care and recognizes the need for a residency program in community care. The vast majority of pharmacy residency programs in existence focus on adult care, while our program is primarily pediatric based. 
 Newly licensed and experienced pharmacists must undergo significant training before they are ready to practice independently in the pediatric setting. Pediatric drug distribution is complex; it encompasses the challenges of adult drug distribution in addition to other intricacies specific to pediatrics. 
 The Department of Pharmacy provides the following services in Ambulatory Care: 
 
 Innovative ambulatory pharmacy services with:
 
 Pharmacists provide pharmacotherapy services in the Primary Care , Close To Home Centers , HIV, Complex Care, and Neurology Clinics 
 Three outpatient pharmacies providing prescription processing and counseling for clinic discharge patients and hospital staff, each with a private consultation room.&#xa0; In addition, a robust discharge counseling service is provided for patients and families.&#xa0; 
 An on-site specialty pharmacy provides medication and clinical services to patients in our Cystic Fibrosis, Rheumatology, Endocrinology, and GI clinics.

								<description>Little Rock, Arkansas,  This residency provides the opportunity to gain clinical experience and provide patient care in a community-based pharmacy practice setting. Residents will gain the tools they need to be leaders in community pharmacy practice with expertise in the development and delivery of patient care services. Residents will work with experienced practitioners and business leaders to implement and study the effectiveness of new programs, enhance existing services and enhance educational skills through didactic and experiential teaching. Residents will also learn how to deliver enhanced clinical services in the community pharmacy setting including, but not limited to, MTM, transitions of care, and chronic care management. 
 Residents will match with one of several sites across the state of Arkansas. Each residency site has a residency site coordinator who serves as the primary point of contact for the practice site. This person oversees the training of the resident and coordinates with other preceptors to ensure learning experiences are progressing. Monthly audio-visual conference calls allows for sharing of practice experiences with co-residents across the state. This program partners with University of Arkansas for Medical Sciences College of Pharmacy, which provides a Teaching Certificate and Research Certificate program to all residents.&#xa0; Applicants must be graduates of an ACPE-accredited Doctor of Pharmacy degree program and be eligible for licensure in the state of Arkansas.

								<description>Fairfax, Virginia,  The Inova Pharmacy Plus PGY1 Community Residency provides clinical and administrative training for pharmacists suitable for entry into an advanced patient care role as a community or ambulatory care pharmacist or matriculation into a PGY2 residency program. We are a new program (currently in candidate status with ASHP) and are excited to offer the opportunity for our resident to help shape our program.&#xa0; Our resident will primarily work at our Inova Schar Cancer Institute specialty pharmacy.  Program overview Experiences include developing, delivering, and assessing continuous quality improvement programs for community pharmacy services; creating, implementing, and evaluating programs to improve patient safety, enhancing educational skills by earning a teaching certificate and engaging in classroom and experiential teaching; communicating and promoting services to various health care providers and educating patients regarding disease prevention and treatment; gain skills in the management of a community pharmacy; and promoting professional development through local and national meetings, participation in pharmacy organizations, and by building professional relationships.&#xa0; The Resident also practices in an ambulatory care clinic one day per week. 
 This program partners with Chicago State University College of Pharmacy, which provides a Teaching Certificate program to all residents.&#xa0; This Residency is based in Chicago, IL.</description>

								<description>Boise, Idaho,  This Albertsons Pharmacy/Idaho State University Program is based in Boise, Idaho, one of the fastest growing metropolitan areas in the US with amenities such as outdoor recreation, riverfront paths, parks, restaurants, museums, and music. Idaho is also a leading state for the advancement of pharmacy practice, and pharmacists can now administer and prescribe a growing list of medications.&#xa0; Each resident is assigned a home-base community pharmacy site where approximately 40% of their time is spent. Each site has a dedicated team of experienced preceptors to mentor the resident through the year. Both sites are located close to the Corporate and Division offices, where leadership and management and specialty care learning experiences take place. Our advanced community-based patient care learning experiences are supplemented with ambulatory care-based patient care with Terry Riley Health Services and St. Alphonsus Regional Medical Center. Through our partnership with Idaho State University, the residents participate in a teaching certificate program and performs research at ISU&#39;s Meridian Campus. Our program&#39;s learning experiences have been specifically designed not only to ensure you have the skillset necessary for success immediately upon graduation, but also in the near-future when pharmacy practice has further advanced. These learning experiences allow room for adjustment based on your strengths, interests, development opportunities, and goals.
