								<description>Royal Oak, Michigan,  Beaumont Hospital, Royal Oak offers a twelve-month American Society of Health-System Pharmacists (ASHP) accredited, PGY2 Ambulatory Care Pharmacy Residency program for one residency candidate. 
 Our program strives to meet the needs of the resident based on previous PGY1 residency experience, interests, and individual goals of the candidate. The residency program is divided into four 3-month block rotations, with the first block focusing primarily on orientation to the &#xa0;Diabetes, Anticoagulation, Discharge Transition Clinic and High-Risk OB Clinics. An individual training plan is created for each resident. Throughout subsequent blocks the resident will rotate through a number of core and elective rotations that serve to develop leadership skills and clinical expertise in Ambulatory Care related pharmacotherapy. All residents complete a research project and present their study results at a regional residency conference in the Spring.&#xa0; 
 The PGY2 Ambulatory Care pharmacy residency program builds on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in advanced or specialized practice. The PGY2 Ambulatory Care residency provides residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care that improves medication therapy. The purpose of the PGY2 Ambulatory Care Pharmacy program is to prepare the resident to obtain a position as an Ambulatory Care Clinical Specialist. Residents will take full responsibility for the care of each patient served.&#xa0; They will provide necessary education for both medical professionals operating in the Outpatient Clinic as well as all patients treated.&#xa0; All pharmacotherapy and disease state issues will be addressed and resolved while taking into account potential barriers in the implementation of the pharmacotherapy plan (i.e. patients&#39; socioeconomic status, insurance status, and/or cultural barriers).&#xa0; Residents will practice pharmacy and communicate to other healthcare professionals in a manner that displays commitment to the profession and importance of a clinical specialist in the outpatient setting.

The Turley Family Health Center is the primary teaching site for the USF Morton Plant Mease Family Medicine Residency Program.  Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available. Applicants to the program are graduates of an accredited pharmacy degree program (or one in process of pursuing accreditation) or have a Foreign Pharmacy Graduate Equivalency Committee certificate from the NABP. Minimum 5-year pharmacy degree program is required.  The&#xa0;Durham VA Health Care System (DVAHCS) PGY2 Ambulatory Care Pharmacy Residency&#xa0;Program is designed to develop a healthcare practitioner with&#xa0;advanced skills to support a specialized practice in the care of ambulatory&#xa0;patients.&#xa0;The practice sites deliver care to veterans following the PACT (patient-aligned care team) model which is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of resources. The PACT model places the veteran at the center of the care&#xa0;team. The care team also includes their family members, caregivers, and health care professionals&#8212;primary care provider, nurse care manager, clinical associate, and administrative clerk.&#xa0;When additional services are needed to meet the veteran&#39;s goals and needs, additional team members may be called in for support. These may include social workers, dietitians, pharmacists, mental health practitioners, specialists, and other non-VA health care professionals. The role of the clinical pharmacy specialist on each team is to design, implement, and monitor therapeutic drug plans to improve patient outcomes through direct interactions with patients and interdisciplinary healthcare team members. The residency program will build upon the competencies and experience acquired in a PGY1 residency and promote the development of proficient, independent clinicians with an expertise in ambulatory care pharmacotherapy. Graduates of the program will develop in-depth knowledge of primary care disease states and the therapies used to treat them. Additionally, upon completion of the PGY2 program, the resident should possess competencies that enable attainment of board certification in ambulatory care.

 Practice Settings 
 The DVAHCS is comprised of the Durham VA Medical Center, Blind Rehabilitation Outpatient Clinic, Brier Creek Dialysis Clinic, Clayton-East Raleigh Community-Based Outpatient Clinic (CBOC), Greenville Health Care Center, Hillandale Road Clinic I, Hillandale Road Clinic II, Morehead City CBOC, Raleigh I CBOC, Raleigh II CBOC, and Raleigh III CBOC. Residents in the PGY2 Ambulatory Care Pharmacy Residency Program will have the opportunity to provide direct patient care at the Raleigh I CBOC, Raleigh III CBOC, Durham VA Medical Center, and Hillandale Road Clinic I. The rotation schedule is developed at the beginning of the residency year based on the resident&#8217;s interests and career goals. The resident will complete all required rotations, one 1-month elective, and one 3-month elective block to allow for continuity in specialty care. The resident will choose 4-6 specialty learning experiences and spend &#xbd; to 2 days per week in each area for 3 months. Additionally, the resident will have one project month dedicated to their research project and other scholarly activities. Schedules may be tailored throughout the year to meet the resident&#8217;s professional goals.

 The Veterans Health Administration sets annual residency stipends (currently $46,613) and benefits for all VA residency programs. Each resident is offered 13 days each of accrued vacation and sick leave/family care, 10 paid federal holidays, and a health care benefits package. Pharmacy residents are also given educational leave with pay to attend and present at local and national conferences. The resident may also be eligible for a faculty appointment with the UNC Eshelman School of Pharmacy.&#xa0; 
 Eligibility Requirements 
 Qualified applicants are required to possess a PharmD degree from an accredited college of pharmacy, be a citizen of the United States, have an active pharmacy license in any state by the first day of the residency, and have completed an ASHP-accredited PGY1 residency training program.

 Saint Joseph Health System is proud to offer two Pharmacy Resident positions in a 52 week, ASHP accredited, PGY2 Ambulatory Care pharmacy residency program. This residency is conducted primarily within Saint Joseph Health System Family Medicine Center which also houses our Family Medicine residency program, as well as primary care offices of our physician network. The residency year is comprised of both elective and required experiences combined in a quarterly format in addition to elective block rotations. Yep, we know it&#39;s nontraditional. Feel free to ask us about it. The curriculum of the PGY2 Ambulatory Care Pharmacy Residency Program emulates the practice of the Pharmacy Residency faculty. 								<description>Royal Oak, Michigan,  Beaumont Hospital, Royal Oak offers an ASHP-accredited&#xa0;twelve-month Critical Care PGY2 Residency program for one residency candidate. Our program strives to meet the needs of the resident based on previous PGY1 residency experience, interests and individual goals of the candidate. 
 This residency program is divided into ten-one month critical care rotations, an orientation month and a research month. An individual training plan is created for each resident. All residents complete a research project and present their study results at a regional residency conference in the spring. &#xa0; 
 PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in advanced or specialized practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care that improves medication therapy. Residents who successfully complete an accredited PGY2 pharmacy residency should possess competencies that qualify them for clinical pharmacist and/or faculty positions and position them to be eligible for attainment of board certification in that practice area (when board certification for the practice area exists). 
 This residency provides a well-rounded experience in both the medical and surgical critical care populations. A number of core and elective rotations strengthen the resident&#8217;s critical care fundamentals as well as providing flexibility to meet the individual&#39;s needs. The program is designed to transition PGY1 residency graduates from general pharmacy practice to specialized practice that meets the needs of critically ill patients by developing the resident&#39;s clinical expertise in critical care. It also provides additional opportunities for teaching, research, quality improvement, and professional writing. 
 Training focuses on developing resident capability to deal with a range of diseases and disorders that occur in the critically ill. Special emphasis is placed on the complexities of multiple organ system failure and the difficulties imposed on care when patients require life-sustaining equipment. 
 At residency completion, PGY2 graduates exit equipped to be fully integrated members of the interdisciplinary critical care team, able to make complex medication recommendations in the acute environment of the critically ill patient population.&#xa0; Graduates of the critical care residency are also experienced in short-term research in the critical care environment and excel in their ability to teach other health care professionals. They also acquire the experience necessary to exercise leadership for critical care practice in the health system. 
 The goal of the program is to develop independent pharmacy practitioners to become integral members of the critical care multidisciplinary team.

The program emphasizes the integrated approach to the application of advanced clinical skills. The PGY2 pharmacy residency program builds on Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experiences and knowledge, and incorporating both in the into the provision of patient care that improves medication therapy. Residents who successfully complete an accredited PGY2 pharmacy residency should possess competencies that qualify them for clinical pharmacist and/or faculty positions and position them to be eligible for attainment of board certification in cardiology pharmacy.

 The specific program for each resident varies based upon the resident&#8217;s goals, interests, and previous experience. All residents are required to complete the above core rotations. The elective rotations are available to provide flexibility in pursuing individual goals.
