								<description>Vancouver, Washington,  PeaceHealth Southwest Medical Center (PHSW)&#xa0;began offering a one-year ASHP-accredited residency in pharmacy practice in July of 1996. When the program began, two residents were selected to participate in the program. After successful completion of the third resident class, the program expanded to the selection of three residents in 1999, four residents in 2007. In 2017, PHSW joined with PeaceHealth St John to offer six residency positions between the two hospital sites. Starting in 2019, each hospital offered&#xa0;their own residency program and PHSW&#xa0;resumed&#xa0;training four residents.&#xa0; PHSW has continued to expand our residency program with the addition of a PGY2 Ambulatory Care Pharmacy residency position.&#xa0;&#xa0;While&#xa0;structured to meet the intent of the current ASHP Post-graduate year one (PGY1) residency program standards, training experiences are individualized based on existing resident competencies and personal interests. 
 The purpose of this residency program is to develop competent clinical practitioners who have accelerated their growth beyond entry-level professional competence in patient care and in practice management, and have further developed leadership skills that may be applied in any direct patient care position in a variety of practice settings upon completion of the residency.  The Broward Health Medical Center (BHMC) Pharmacy Practice Residency Program is accredited by The American Society of Health System Pharmacists. It is&#xa0;a 12 months full-time practice commitment program, utilizing various modalities to help the pharmacy resident become a well-rounded clinical practitioner.&#xa0;BHMC Pharmacy Residents will participate in operational, clinical, and administrative pharmacy experiences.  Our ASHP-accredited PGY1 Pharmacy Practice Residency program at Kettering Medical Center (will be Kettering Health Main Campus) is a 12-month post-graduate program that focuses on developing a competent, confident, and well-rounded pharmacist in the acute care setting. The program offers a variety of experiences and has equipped residents for positions in specialized clinical roles, PGY2 training, academia and others. Kettering Medical Center has been graduating well-trained PGY1 pharmacy residents since 2013. Day to day, residents are directly involved in providing pharmaceutical care to patients as part of the multidisciplinary team in a variety of practice areas including internal medicine, pulmonary critical care, trauma/surgical critical care, emergency medicine, inpatient oncology, antimicrobial stewardship, neonatal intensive care and cardiology. The PGY1 Resident will be trained in Pharmacy Consult Services including pharmacokinetic dosing and monitoring, nutrition support, anticoagulation, and antiarrhythmic medication patient counseling. Development of pharmacy leadership skills in both the inpatient administrative and operational realms is also a priority.

 Our ASHP-accredited PGY1 Pharmacy Practice Residency program at Grandview Medical Center (will be Kettering Health Dayton) is a 12-month post-graduate program that focus on developing a competent, confident, and well-rounded pharmacist in the acute care setting. The program offers a variety of experiences and has equipped residents for positions in specialized clinical roles, PGY2 training, academia and others. Grandview Medical Center has been graduating well-trained PGY1 pharmacy residents since 2010. Day to day, residents are directly involved in providing pharmaceutical care to patients as part of the multidisciplinary team in a variety of practice areas including internal medicine, critical care, emergency medicine, hematology/oncology, antimicrobial stewardship, neonatal intensive care and cardiology. &#xa0;The PGY1 Resident will be trained in Pharmacy Consult Services including pharmacokinetic dosing and monitoring, nutrition support, anticoagulation, and antiarrhythmic medication patient counseling. Development of pharmacy leadership skills in both the inpatient administrative and operational realms is also a priority.

								<description>Dublin, Georgia,  This is an organized ASHP accredited postgraduate year one (PGY1) pharmacy residency program directed to focus on the development of knowledge, attitude, and skills needed to become a competent clinical practitioner. 
 The purpose of this PGY1 pharmacy residency program is to build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. 
 This residency program aims to train pharmacists to provide pharmaceutical care, primarily to ambulatory, geriatric, mental health and long term care patients, participate as a member of the health care team, and provide education of patients, and health professionals. In addition, residents will learn to perform self-monitoring and demonstrate leadership through contributions to performance improvement. Residents will be encouraged to develop an approach to the profession that can lead to life-long learning and career satisfaction. 
 The Carl Vinson VA Medical Center (CVVAMC), located in Dublin, Georgia. &#xa0;The CVVAMC also manages six community-based outpatient clinics (CBOCs) located in Albany, Brunswick, Macon, Milledgeville, Perry &#38; Warner Robbin Annex, and Tifton, Georgia. &#xa0; &#xa0;The CVVAMC serves 49 counties in middle and south Georgia, and treats more than 37,000 Veterans with over 200,000 visits annually. The largest employer in Laurens County, GA, with over 1,300 employees, the medical center also attracts thousands of Veterans and guests to the area, as well as hundreds of contractors. A primary and secondary care facility, the CVVAMC has 340 beds for medical and surgical care, 161 community-living center (formerly called &#8220;nursing home&#8221;) beds, and 145 domiciliary beds for the residential treatment that includes programs to deal with Veteran homelessness, substance-abuse, and post-traumatic stress disorder (PTSD). In addition to the services expected in a modern medical center, such as primary care, diagnostic, pharmacy, mental health, and acute care, the medical center offers specialty clinics, such as audiology, podiatry, optometry, and other programs that include Home-Based Primary Care (HBPC), HUD-VASH vouchers to address Veteran homelessness, social work, pulmonary, physical therapy, cardiology, orology, and palliative care. An integral part of the Dublin-Laurens community, medical center staff participate in community events including parades and St. Patrick&#8217;s Day festivities, and the CVVAMC maintains affiliations with a number of colleges and universities.
