 Rhode Island Hospital is a world-class academic and research facility: the state&#39;s largest acute care hospital, a level-1 trauma center and the principal teaching hospital for Brown University&#39;s medical school as well as the recipient of the &quot;100 Most Wired&quot; Award for three consecutive years. We are affiliated with the University of Rhode Island, College of Pharmacy.  Purpose 
 Our PGY2 residency programs provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academia, or other specialized positions, along with board certification. 
 The oncology residency program provides comprehensive experiences in hematology and oncology clinical pharmacy practice. Experiences include providing pharmaceutical care to acute and ambulatory (adult and pediatric) hematology/oncology patients, investigational drug services, palliative care, and a variety of elective and longitudinal rotations.&#xa0;Residents will also function as adjunct faculty for pharmacy students on rotation and in didactic learning experiences at the College of Pharmacy.&#xa0; 
 The ambulatory care residency program&#xa0;provides an opportunity to develop the necessary skills for specialized practice in ambulatory care clinical&#xa0;pharmacy.&#xa0;Residents&#xa0;will be directed in skills critical to patient care, communication and leadership along with&#xa0;comprehensive experiences involving collaborative practice and medication therapy management in a variety of practice settings.&#xa0;Practice settings include, but are not limited to, adult primary care, diabetes mellitus, dermatology, gastroenterology, rheumatology, specialty pharmacy, transitions of care and renal transplantation.&#xa0;Residents will also function as adjunct faculty for pharmacy students on rotation and in didactic learning experiences at the College of Pharmacy.

 Purpose 
 PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification and eligible for postgraduate year two (PGY2) pharmacy residency training&#xa0; 
 Pharmacy Services 
 Coastal Medical has a unique and innovative pharmaceutical care program with over 10 clinical pharmacists working in an Accountable Care Organization. Clinical pharmacy services are provided to all of the adult internal medicine practices, family medicine practices and the sub-specialty practices including cardiology and pulmonology.&#xa0;Pharmacists are a part of Coastal Medical&#8217;s population health strategy and integral members of our care management programs. Pharmacists&#xa0;work under a collaborative practice agreement with all of the Coastal Medical providers.&#xa0; The collaborative practice agreement scope of practice includes diabetes, cardiology, respiratory diseases, smoking cessation, HIV/AIDS and general medication therapy management.&#xa0;In addition, the team provides opioid and antibiotic stewardship services, conducts comprehensive medication reviews for targeted patients via OutcomesMTM, and provides drug information services to the providers. Members of the team also participate in multidisciplinary care conferences. Patients may be referred to telephonic or face to face services by any member of the healthcare team or patients may self-refer.&#xa0; Consultations are offered to patients at no charge.&#xa0;The pharmacists document all patient interactions in the electronic health record which is shared and viewable by each member of the healthcare team.&#xa0; 
 In the pharmacy program, pharmacists have several unique roles including office-based pharmacists,&#xa0;population health management pharmacists,&#xa0;or managed care pharmacist. Office-based pharmacists are assigned to an office depending on the day of the week to provide clinical pharmacy services, opioid stewardship, and antibiotic stewardship. Population health management pharmacists provide services to patients in the diabetes management program, Coastal@Home program, heart failure program, and chronic obstructive pulmonary disease (COPD) program.&#xa0;The managed care pharmacist works with payers to meet all medication related quality measures and outcomes.&#xa0;The managed care pharmacist also supervises the pharmacy technician team and the medication refill protocol. All pharmacists also participate in the education and training of advanced pharmacy practice experience (APPE) students from the University of Rhode Island.&#xa0; 
 Program Overview 
 The Coastal Medical/URI residency is a unique 12-month PGY-1 residency providing direct patient care in outpatient primary care practices. The resident works collaboratively as a member of the healthcare team to prevent and alleviate medication-related problems, optimize patient outcomes, advance the workflows for medication use and safety in primary care, and evaluate the impact of medication management services. The resident will receive training in the delivery of direct patient care, the enhancement of professional communication and interpersonal skills, academic teaching, student precepting, and management skills. Clinical experiences may include adult internal medicine, diabetes, cardiology, pulmonology, smoking cessation and pain management. The resident will have the opportunity to complete a medication class review and provide drug information services while on rotation at the URI Drug Information Services. 
 Our student precepting environment allows residents to develop strong teaching skills. Our capable research staff is an excellent resource for assisting the resident in developing a solid foundation in research design and analysis.&#xa0;

 The resident will meet quarterly with the residency program director to tailor the resident&#8217;s schedule to meet individual goals. Upon completion of the program, the resident will be awarded an ASHP accredited residency certificate if all program requirements are met. 
 The resident will be required to attain a Rhode Island professional pharmacy license within 90 days of the start date of the residency. Failure to attain this license can result in the termination of employment with the residency program. 
 Program Benefits 
 The resident will receive a 1-year stipend in the total amount of $43,500. The resident is eligible for Coastal Medical health insurance coverage. 10 vacation days and 5 sick days are available. A travel reimbursement will be provided to attend a clinical meeting to present the resident&#8217;s research project. The resident is also provided with a dedicated workspace and a laptop computer.

								<description>Royal Oak, Michigan,  Beaumont Hospital, Royal Oak offers a twelve-month American Society of Health-System Pharmacists (ASHP) accredited, PGY2 Ambulatory Care Pharmacy Residency program for one residency candidate. 
 Our program strives to meet the needs of the resident based on previous PGY1 residency experience, interests, and individual goals of the candidate. The residency program is divided into four 3-month block rotations, with the first block focusing primarily on orientation to the &#xa0;Diabetes, Anticoagulation, Discharge Transition Clinic and High-Risk OB Clinics. An individual training plan is created for each resident. The PGY2 Ambulatory Care residency provides residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care that improves medication therapy. The purpose of the PGY2 Ambulatory Care Pharmacy program is to prepare the resident to obtain a position as an Ambulatory Care Clinical Specialist. Residents will take full responsibility for the care of each patient served.&#xa0; They will provide necessary education for both medical professionals operating in the Outpatient Clinic as well as all patients treated.&#xa0; All pharmacotherapy and disease state issues will be addressed and resolved while taking into account potential barriers in the implementation of the pharmacotherapy plan (i.e. patients&#39; socioeconomic status, insurance status, and/or cultural barriers).&#xa0; Residents will practice pharmacy and communicate to other healthcare professionals in a manner that displays commitment to the profession and importance of a clinical specialist in the outpatient setting.

 The&#xa0;Durham VA Health Care System (DVAHCS) PGY2 Ambulatory Care Pharmacy Residency&#xa0;Program is designed to develop a healthcare practitioner with&#xa0;advanced skills to support a specialized practice in the care of ambulatory&#xa0;patients.&#xa0;The practice sites deliver care to veterans following the PACT (patient-aligned care team) model which is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of resources. The PACT model places the veteran at the center of the care&#xa0;team. The care team also includes their family members, caregivers, and health care professionals&#8212;primary care provider, nurse care manager, clinical associate, and administrative clerk.&#xa0;When additional services are needed to meet the veteran&#39;s goals and needs, additional team members may be called in for support. These may include social workers, dietitians, pharmacists, mental health practitioners, specialists, and other non-VA health care professionals. The role of the clinical pharmacy specialist on each team is to design, implement, and monitor therapeutic drug plans to improve patient outcomes through direct interactions with patients and interdisciplinary healthcare team members. The residency program will build upon the competencies and experience acquired in a PGY1 residency and promote the development of proficient, independent clinicians with an expertise in ambulatory care pharmacotherapy. Graduates of the program will develop in-depth knowledge of primary care disease states and the therapies used to treat them. Additionally, upon completion of the PGY2 program, the resident should possess competencies that enable attainment of board certification in ambulatory care.

 The DVAHCS is comprised of the Durham VA Medical Center, Blind Rehabilitation Outpatient Clinic, Brier Creek Dialysis Clinic, Clayton-East Raleigh Community-Based Outpatient Clinic (CBOC), Greenville Health Care Center, Hillandale Road Clinic I, Hillandale Road Clinic II, Morehead City CBOC, Raleigh I CBOC, Raleigh II CBOC, and Raleigh III CBOC. Residents in the PGY2 Ambulatory Care Pharmacy Residency Program will have the opportunity to provide direct patient care at the Raleigh I CBOC, Raleigh III CBOC, Durham VA Medical Center, and Hillandale Road Clinic I. The rotation schedule is developed at the beginning of the residency year based on the resident&#8217;s interests and career goals. The resident will complete all required rotations, one 1-month elective, and one 3-month elective block to allow for continuity in specialty care. The resident will choose 4-6 specialty learning experiences and spend &#xbd; to 2 days per week in each area for 3 months. Additionally, the resident will have one project month dedicated to their research project and other scholarly activities.  The Veterans Health Administration sets annual residency stipends (currently $46,613) and benefits for all VA residency programs. Each resident is offered 13 days each of accrued vacation and sick leave/family care, 10 paid federal holidays, and a health care benefits package. Pharmacy residents are also given educational leave with pay to attend and present at local and national conferences. The resident may also be eligible for a faculty appointment with the UNC Eshelman School of Pharmacy.&#xa0;

 The Division of Pharmacy at The University of Texas MD Anderson Cancer Center offers a one-year, ASHP-accredited PGY2 Critical Care pharmacy residency program. MD Anderson has 650+ inpatient beds, including 50+ ICU beds with provision of care by full-time intensivists, acute care advanced practice providers, clinical pharmacy specialists, and other critical care personnel. The residency program is heavily weighted in direct patient care and designed to facilitate development of clinical expertise and competence in critical care pharmacotherapeutics. The residency learning experiences include both core ICU rotations as well as two months of elective time that will be tailored to the interests and career goals of the resident. Through these experiences as well as research, publication, presentation, ACLS, and precepting requirements, the resident will develop the skills necessary to become a successful and comprehensive Critical Care clinical pharmacy specialist.
