PGY2 Ambulatory Care Pharmacy Residency (Mount Sinai Health Partners)
This PGY2 ambulatory care residency program has been designed to foster the development of a clinician that is able to apply evidence-based medicine within various population health models or other advancing ambulatory care pharmacy settings. This will be achieved through interdisciplinary and independent learning opportunities in a variety of diverse clinical areas. Strengths of this program include longitudinal patient-centered care within interdisciplinary teams, numerous opportunities in digital medicine, population health, management, and research. A focus will be on refining skills to manage chronic disease states (with an emphasis on diabetes and hypertension), transitions of care, medication reconciliation, and care management.
