Utilize PhORCAS to submit the following:

• Letter of Intent
• Curriculum Vitae
• 3 Recommendations – 1 must be from PGY-1 Residency Program Director
• Official Transcripts
  • If from a pass/no pass College of Pharmacy, send the class ranking directly to the program director, in addition to transcripts.

A 15-20 minute (excluding question and answer) presentation on a critical care topic is required. Other types of presentations (e.g., journal club, monograph) are not appropriate. The presentation must be emailed to the residency program director at least 1 week prior to the interview date.
