This program provides training, mentoring, and evaluation for pharmacists seeking additional education in a wide variety of ambulatory care areas, including primary care, cardiology, anticoagulation, diabetes/weight management, geriatrics, and transitions of care. At the beginning of the year, residents will follow a block schedule to develop the clinical skills needed to provide patient-focused care. In the second half of the year, residents will follow a longitudinal schedule to deliver pharmaceutical care in a variety of settings, leading to stronger relationships with patients and the healthcare team. The program's unique staffing component is focused on transitions of care, mainly at hospital discharge. The program provides experiences relevant to current health care needs and offers opportunities to effectively teach and evaluate the learning needs of pharmacy students. Our institution also offers a PGY2 in ambulatory care. The opportunity to early commit for this program is available.
We are using the ASHP-supported application portal, PhORCAS, for residency application submissions. Applications must include a letter of intent, CV, college/pharmacy school transcripts, and 3 letters of recommendation using the standardized form on PhORCAS. *The application deadline for Mission Hospital is Friday, January 2, 2026* The preference is to have residents licensed by the start of residency. Residents must be licensed within 90 days of starting the program.
