The postgraduate year two (PGY2) ambulatory care pharmacy residency was founded in 2015 and offers three positions at Community Health Network annually. This residency is flexible to help meet your professional goals with opportunities in academia, new service development and clinical practice experience in a variety of specialty areas. This program provides well-rounded training to prepare residents to become clinical ambulatory care pharmacists in any setting, including academia. Community Health Network’s Ambulatory Care Pharmacy Department consists of over 45 clinical ambulatory care pharmacists practicing throughout the greater Indianapolis area in a variety of clinics, including primary care, cardiology, infectious disease, transitions of care and women’s health. Pharmacists work closely with physicians and healthcare teams to optimize medication regimens in both primary care and specialty disease states. Our extensive collaborative drug therapy management agreement has been signed by over 200 physicians and covers 20 protocols, including anticoagulation, asthma, continuous blood glucose monitoring, contraception, COPD, diabetes, dyslipidemia, GERD, HIV pre-exposure prophylaxis, hypertension, hypothyroidism, Medicare Annual Wellness Visits, naloxone assessment, nicotine cessation, nirmatrelvir-ritonavir (Paxlovid™), osteoporosis, pharmacotherapy review, seasonal allergies, weight management, and vaccinations.
