Anticoagulation Stewardship is a current growth area in specialized pharmacy practice. To help facilitate the growth of Anticoagulation Stewardship programs nationally, the Anticoagulation Forum and the American Society of Health-System Pharmacists have collaborated in launching a newly recognized PGY2 specialty area of Thrombosis and Hemostasis Management. The 2026-2027 academic year will be the fourth year we offer this residency opportunity. Our program offers a combination of block learning experiences and longitudinal learning experiences that occur over the course of the residency year. Required block experiences include orientation, inpatient anticoagulation 1 and 2, cardiology, critical care, and emergency medicine. Longitudinal experiences include outpatient anticoagulation, pathology/anticoagulation laboratory science, anticoagulation stewardship leadership, thrombosis and hemostasis medication safety, service, and research. Elective block rotations in pharmacogenomics and benign hematology are available. Residents will be required to complete a research project over the course of the year and will attend a national meeting, SDSHP Annual Meeting, and the Midwest Pharmacy Residency Conference, which are all part of the residency program.

Sanford USDMC provides an exciting learning environment to disciplines including medicine, pharmacy and nursing. PGY1 Pharmacy: 4 positions PGY2 Clinical Pharmacogenomics: 1 position PGY2 Thrombosis and Hemostasis Management: 1 position. Resident stipends are competitive and updated annually. Residents are provided a health and vision insurance stipend, and are allotted 20 days of Allowed Time Away (ATA). All required travel expenses are covered.
