Program Description:

Extensive clinical pharmacy services within an integrated department provide PGY1 Pharmacy Residents with active preceptorship in ambulatory care (primary care, infectious diseases or psychiatry), critical care (burn, medical/pulmonary or trauma/surgical), internal medicine, infectious diseases (antimicrobial stewardship or acute care consults), cardiology, clinical informatics, emergency medicine, general surgery/anesthesia, pain and opioid stewardship, oncology/hematology, and inpatient psychiatry. Residents participate in the Indiana Pharmacy Teaching Certificate Program and have didactic and/or experiential teaching opportunities with Purdue University College of Pharmacy or Butler University College of Pharmacy. Residents develop mastery of pharmacy operations through inpatient staffing every third weekend and develop leadership and project management skills through research, practice management, medication use process and committee co-chair experiences.

All applications will be completed using PhORCAS.  We suggest you submit program specific evaluations through PhORCAS as we require program specific information to be included by both the applicant and the recommenders.  Please see specific information under the application section of our website.

-Updated curriculum vitae

-College/School of Pharmacy transcripts

-Responses to supplemental application questions instead of a letter of interest/intent.

-3 Recommendations using the standardize recommendation form AND responses to our supplemental recommendation form.  This information can be loaded in the program specific content area of the standardized reference form and should be done in place of a formal letter of recommendation.

-Video interview between 12/27/21 and 1/3/22 as part of the initial application process.  Information will be emailed to applicants once the application deadline has closed.

Requirements:

Enrolled in or graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited degree program (or candidate status) or have a Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate from the National Association of Boards of Pharmacy (NABP).

Official college/school of pharmacy university transcript GPA of at least 3.0.  If applicant is from a school with pass/fail designation only, the applicant must have a letter from the college listing his/her ranking and must be within the top 50% of the class.
