About Our Program:
This is a PGY1 ambulatory care focused pharmacy residency, located in State College, PA. The resident will work in multidisciplinary primary care and specialty practices and collaborate with team members utilizing collaborative practice agreements to implement safe, efficacious and cost-effective drug therapy regimens. Upon establishing competency in specific disease states, the resident will longitudinally provide medication management services for patients with chronic diseases both inperson and via telehealth. The residency runs longitudinally to enable residents to develop relationships with patients, establish a patient panel, and recognize impact of their clinical decision-making. Specific learning experiences are included below. Each resident’s curriculum is customized to facilitate achievement of their personal and professional goals.
Internal Medicine with Transition of Care Focus (3 weeks)
Required Core Longitudinal Learning Experiences:
Family Practice Medication Therapy and Disease Management (~8 month experience - 2 days per week)
Geriatric Medication Therapy Disease Management (~4 month experience – 1.5 days per week)
Cardiology (~4 month experience - 1.5 days per week)
Staffing/On Call (10 months - 1/2 day per week and every other weekend on call)
Required Longitudinal Learning Experiences:
Pharmacy Administration and Medication Safety (10 month experience - 0.5 day every other week)?
Drug Information and Evidence Based Medicine (11 month experience–1 hour every other week and as needed)
Practice Advancement Project (11.5 month experience and as needed)
Leadership and Teaching (10 month experience?– 1 hour every other week and as needed)
Elective Learning Experiences: Residents have the opportunity to select 1 elective which will be approximately 4 weeks in length; length may be modified based on Resident interest and preceptor availability except where noted. Residents will also be offered the opportunity to complete a 2-week career prep elective rotation pending achievement of all R1 objectives in other learning experiences

Program Requirements:
Completion of all R1 goals/objectives with a status of Achieved for Residency by the end of the residency. Completion of 100% of the remaining goals/objectives for the program with at least 75% of the objectives having a status of Achieved for Residency. All objectives must have a minimum status of satisfactory progress. Individual programs may require a higher minimum expectation but may not require less than identified as part of this policy. Programs with a higher minimum requirement will communicate this to residents during the interview, contracting, and orientation process.
All evaluations must be completed in accordance with ASHP Accreditation Standard as outlined in the Resident Agreement
Completion of all certifications by the dates defined by the residency program director and/or system policy.
Completion of major project, which includes meeting all project deadlines and submission of written manuscript of completed project to the RAC.
Assigned projects (e.g., practice advancement project, drug review, management project) have been completed.
Presentation of at least one major CME program.
Poster or platform presentation at one or more regional or national professional meetings:
Poster presentation at ASHP Midyear Clinical meeting & platform presentation at Eastern States Conference
Completion of minimum staffing requirements as described in residency contact/manual.
Adherence to the organization’s attendance standards.
Application Requirements:
The program will accept all applications through the Pharmacy Online Residency Centralized Application Service (PhORCAS) and participate in the national match. The applicant must submit the following materials via PhORCAS:
Letter of intent outlining career objectives
Curriculum vitae
Academic transcripts
Three professional recommendations
Also required is a Doctor of Pharmacy or equivalent degree from an Accreditation Council for Pharmacy Education (ACPE) accredited school or college. Applicants with certification from the Foreign Pharmacy Graduate Examination Committee (FPGEC) are also welcome to apply. Candidates must be licensed or eligible for pharmacist licensure in the Commonwealth of Pennsylvania. Licensure requirements are available at the Pennsylvania State Board of Pharmacy website. Applicants should ensure that they meet all the requirements for licensure in the Commonwealth of Pennsylvania. Licensure is required within the first 60 days of the residency program.
The deadline for application is January 2nd, 2026. Individual interviews are required and are scheduled by invitation. Interviews will be conducted in a virtual setting. Applications are reviewed in the order in which they are received. All candidates will be notified regarding the status of their application by January 30th, 2026.
The Geisinger Clinic Pharmacy program has more than 70 ambulatory clinical pharmacists managing patients’ medications for over 20 medical conditions. Pharmacists are incorporated into clinical sites within four practice models: primary care disease management, specialty medicine disease management, home-based primary care and pharmacy tele-management. Pharmacy students from regional schools also rotate through clinics as part of their advanced pharmacy practice experience rotations. Residents will therefore have teaching and precepting experiences.
